The purpose of Support Coordination is to assist participants to implement an approved plan by strengthening their ability to connect with the broader systems of support and understanding the purpose of the funding.
Support Coordinator’s will assist a participant to understand all aspects of the plan, aid in the ongoing management and answer questions as they arise.
Our experienced Support Coordination staff can act on behalf of the participant, to contact organisations, community groups and services to research your options and obtain quotes so you can make an informed decision about which services you would like to purchase with your NDIS funding.
You may find that your Support Coordination funding is allocated to Psychosocial Recovery Coaching. This is a newer line of support under NDIS aimed toward participants living with a psychosocial disability who require support to manage complex daily living challenges.
Psychosocial Recovery Coaches will focus on engaging with and encouraging participants toward achieving a greater level of participation and independence in managing their everyday lives. This is done using strategies informed by a knowledge and understanding of psychosocial conditions and built-in collaboration with the participants other supports and allied health professionals to achieve the best possible outcome.
Recovery coaches work closely with participants, their families and other formal supports to develop these strategies as part of a recovery plan designed and implemented with the individual needs and goals of the participant in mind.
This means that upon your sign up with Farris Care Services you will be allocated a Support Coordinator and/or a Psychosocial Recovery Coach with the appropriate skills and wealth of experience to enable you to build a relationship/s centred on trust and ensure you feel supported in working toward your goals.
Our team at Farris Care Services are passionate and resourceful professionals that specialise in Mental Health, Psychosocial and Physical Disability. We work collaboratively with you and all services involved in your care — this can include not just your NDIS services but also your GP or health related services, TAFE or even the Department of Housing to ensure you are linked with the highest quality providers and services to support your needs and are being appropriately advocated for.
We believe in the importance of informed choice and take the time to educate you when needed about the supports that can be funded by your specific NDIS plan and how it can be best implemented to get you the most out of your plan. We develop a comprehensive goal orientated plan in collaboration with you to ensure that from the start of your plan until the end, your supports are working toward helping you achieve what is most important to you.
